Our Physical Therapists- - Evaluate patient and caregiver safety in the home, regarding ambulation, transfers and lifting; in order to identify adaptive equipment needs.
- Make home safety recommendations to reduce the risk of injury, to patient and caregivers.
- Participate, as part of a team, to provide treatment which will maximize patient independence, and minimize risk of injury.
- Develop a realistic home exercise program, in writing, which the patient can perform to achieve his or her own sense of accomplishment on a daily basis.
- Develop and administer a treatment plan designed to achieve the patient’s goals.
Our Occupational Therapists- - Evaluate a patient’s level of safety and independence in their home.
- Recommend home modifications to increase safety with Activities of Daily Living (ADL)
- Provide therapeutic interventions, to include ADL training, functional transfer training, safety awareness training, and adaptive equipment training, to increase a patient’s independence with ADL’s.
- Determine appropriate adaptive equipment needs to assist with increasing patient independence.
- Provide home exercise programs to increase a patient’s upper extremity strength and/or to maintain upper extremity range of motion.
- Provide family education, regarding how to encourage patients to participate in ADL’s, and how to encourage carryover of home exercise programs.
- Determine upper extremity splinting needs and help patient or family to obtain splints as appropriate.
Our Speech Therapists- - Provide evaluations, and appropriate testing and treatments, for patients with speech and/ or language disorders, cognitive deficits, and swallowing disorders.
- Evaluate swallowing dysfunctions and modify patient’s diet accordingly.
- Teach the patient and family how to reduce the risk of aspiration.
- Maximize communication by improving patient’s sound production along with work clarity.
- Work with the patient to improve their safety with eating and swallowing.
